Incidence and Risk Factors of GVHD

Bart L. Scott, MD: There are 2 main types of graft-versus-host disease [GVHD], acute and chronic. Acute typically occurs between day 28 to day 100. For matched unrelated donors, the incidence is generally around 60%, and for matched sibling donors it’s generally around 40%.

What are some of the risk factors for acute graft-versus-host disease? One would be the donor type. If you have a matched sibling donor versus a mismatched unrelated donor, there are higher rates of GVHD with mismatched donors. Another would be the stem cell source. Is it peripheral blood versus marrow? There are higher rates of chronic GVHD, for instance, for patients who get peripheral blood versus bone marrow, with bone marrow having lower rates of chronic GVHD. And then stem cell dose, in general the higher the stem cell dose, the more likely you are to have issues with graft-versus-host disease.
