Real-World Data Expand Evidence for Mogamulizumab in CTCL

Fact checked by: Sabrina Serani

Real-world German MINT data show mogamulizumab delivers durable responses, ~20-month PFS, manageable safety, and improved quality of life in MF and Sézary syndrome.

Mogamulizumab (Poteligeo) was associated with durable real-world treatment outcomes in patients with mycosis fungoides (MF) and Sézary syndrome (SS), according to final results from the German MINT study.1

In this real-world analysis, treatment with mogamulizumab was associated with an overall response rate of 62.4% and a median progression-free survival (PFS) of 20.2 months. Among 109 patients, the median time to next treatment was 14.9 months, providing additional data on treatment duration in routine clinical practice.

“For patients living with [MF] and [SS], understanding how treatments perform outside the controlled setting of clinical trials is critically important,” Chalif Assad, MD, of Helios Klinikum Krefeld, Health and Medical University Düsseldorf-Krefeld in Germany, stated in a news release. “The final MINT results demonstrate meaningful clinical outcomes across a broad patient population treated in routine practice and highlight the value of collaborative real-world research in helping clinicians make informed treatment decisions.”

About the MINT Study

MINT (Mogamulizumab in a Non-interventional Study to Evaluate the Safety and Outcome in Mycosis Fungoides and Sézary Syndrome Treatment) was a multicenter, noninterventional study conducted at 18 centers across Germany. The analysis included 56 patients with MF and 53 with SS, with a median follow-up of 25.2 months. Most patients (83.5%) had advanced disease, defined as stage IIB or higher, at treatment initiation.

Additional Findings: Safety and Quality of Life

Grade 3 or higher treatment-related adverse events (TRAEs) occurred in 38.5% of patients, with drug eruption and lymphopenia among the most commonly reported events. Serious TRAEs leading to treatment discontinuation occurred in 22.0% of patients. The overall safety profile was described as consistent with findings from the phase 3 MAVORIC trial (NCT01728805), which supported mogamulizumab’s FDA approval in CTCL in August 2018.2

Quality of life improved during treatment, with median Skindex-29 scores decreasing from 49.1 at baseline (n = 21) to 30.2 at month 12 (n = 7), representing a 38.5% improvement. However, the small number of patients with available month 12 assessments limits interpretation of this finding.

MINT Findings in the Context of MAVORIC

The MINT findings complement results from MAVORIC, which compared mogamulizumab with vorinostat (Zolinza) in 372 patients with previously treated MF or SS. In MAVORIC, median PFS was 7.7 months with mogamulizumab vs 3.1 months with vorinostat (HR, 0.53; 95% CI, 0.41-0.69; P <.0001).3

Together, the studies contribute to the evidence base for mogamulizumab in MF and SS, with MINT extending observations to a real-world population that included a high proportion of patients with advanced disease. However, the observational design and limited sample sizes in certain subgroups underscore the need for caution when interpreting these findings.
