The Consequences of Practice Gaps in Chronic GVHD Surveillance

Laila Alkhouli, MD, of Cleveland Children's Clinic outlines critical findings from research evaluating how care teams across centers screen, document, and score chronic graft-vs-host disease (cGVHD) in post-transplant patients. Although clinicians generally acknowledge the necessity of monitoring for cGVHD, the study reveals significant practice variability, inconsistent longitudinal follow-up, and a widespread lack of standardized assessment tools across pediatric transplant programs.

The research highlights major gaps in screening frequency and diagnostic rigor over time. Although half of surveyed clinicians conducted monthly cGVHD screening during the first 6 months following day 100 post-transplant, only 37% maintained quarterly assessments beyond 1 year. Standardized patient-reported outcome measures remain rarely implemented, with only 10% of centers utilizing validated tools such as the Lee Symptom Scale. Although most providers query general symptoms like cutaneous itching or dryness, only 12% perform comprehensive skin examinations, and only 21% utilize structured skin scoring tools. While most centers obtain baseline pulmonary function tests at 1 and 2 years post-transplant, marked inconsistency exists regarding intermediate monitoring during the crucial 1-to-2-year window.

Alkhouli emphasizes that the primary issue is not individual provider oversight, but rather the absence of a uniform, nationwide screening protocol. Missing early cGVHD manifestations carries severe clinical consequences for transplant survivors. Irreversible end-organ damage in organs such as the skin, joints, and lungs frequently develops when cGVHD goes undetected during gaps in routine surveillance. Once advanced tissue damage occurs, therapeutic options become minimally effective, causing significant long-term morbidity and elevated mortality. This makes prioritizing multi-organ cGVHD surveillance protocols across all transplant centers essential to catch subclinical changes early, initiate prompt intervention, and prevent irreversible functional impairment.
