Jiaxin (Jason) Niu, MD, and Amanda Edmond, PA-C, open a virtual tumor board on first-line treatment for EGFR-mutated metastatic non–small cell lung cancer (NSCLC). Dr. Edmond presents the first clinical scenario: a 58-year-old former smoker with a right upper lobe mass, mediastinal lymphadenopathy, 2 small bone lesions, and an 8-mm left frontal brain lesion, whose tumor harbors an EGFR L858R mutation with PD-L1 expression of 10%. His goal is to live as long as possible while continuing to work and stay active. Dr. Niu and Dr. Edmond frame the discussion around 3 themes: selecting first-line therapy, preventing and managing treatment-related adverse events, and how co-mutations, such as TP53, factor into first-line decision-making, across 2 patient cases.
