
Translating Myeloma Genetics to Clinical Practice: The Legacy of the CoMMpass Study

Hearn Jay Cho, MD, PhD, chief medical officer of the Multiple Myeloma Research Foundation (MMRF), highlights the transformative impact of the landmark CoMMpass study, an innovative project launched in 2011 that fundamentally reshaped the understanding and clinical management of multiple myeloma. Prior to CoMMpass, there was a profound lack of comprehensive genomic data detailing tumor biology and a minimal understanding of how specific genetic features correlated with the natural history of the disease, initial treatment response, remission duration, and overall survival.
CoMMpass enrolled over 1,000 newly diagnosed, treatment-naive patients, systematically collecting baseline tumor and blood specimens while tracking clinical outcomes through real-world standard therapies. By bridging tumor biology with longitudinal patient data for the first time, researchers identified approximately a dozen distinct molecular subtypes of multiple myeloma. Although most subtypes correspond to standard-risk disease with favorable treatment responses, specific genetic mutations and structural chromosomal abnormalities were mapped directly to high-risk disease characterized by aggressive progression, lower response rates, and significantly shorter remissions.
This molecular characterization definitively defined high-risk multiple myeloma, transitioning the concept from an emerging hypothesis to a precise diagnostic standard used today. The data generated by CoMMpass directly informs risk stratification and routine decision-making at diagnosis. Diagnostic workups utilize bone marrow genetics to differentiate standard-risk from high-risk patients. Clinicians leverage these insights to tailor therapeutic strategies, recommending standard regimens for low-risk disease while directing high-risk patients toward more aggressive therapies, stem cell transplantation, or novel clinical trials. Ultimately, by tying molecular genetics directly to real-world patient outcomes, CoMMpass established a precision oncology framework that continues to optimize individual treatment plans and guide clinical research,
